Need Records or Other Documents?

If you need copies of your records or other papers, our team at reTHINK Therapy is here to help! We know it’s important to have the information you need for school, work, legal stuff, or just for yourself. We’ll guide you through the steps and make sure your information stays private and accurate.

You can ask for things like FMLA forms, disability paperwork, medical records, proof that you came to therapy, or a summary of your treatment. People need these documents for lots of reasons, like taking time off work, sharing information with a doctor, or showing proof of care. Because these documents take time to create, both our front office and your therapist help put them together carefully.

Here’s how to ask for your records or documents:

Request Process

1. Fill Out the First Form

Start by filling out the Initial Documentation Request Form. Once you turn it in, our Office Manager gets a message right away. This form tells us what you need so we can get started.

2. We’ll Send a Price Estimate

Within 1–2 business days, the Office Manager will send you a message with the price. Most requests cost between $30 and $60, depending on what you’re asking for. If you have questions about the price, we’re happy to explain.

3. Approve and Pay

If the price looks okay to you, we’ll charge your card on file.

We can’t start working on your request until payment is made. Paying ahead of time helps us stay on schedule and make sure we focus on your request right away.

4. Fill Out the Final Form

After we get your payment, you’ll get another form to fill out through TherapyPortal. This one asks for more details, like what dates you need or who the papers are going to. The more info you give us, the better we can help.

5. We’ll Get to Work

Once we have your payment and final form, we’ll start putting your documents together. It usually takes 5–7 business days. If it’s going to take longer, we’ll let you know.

Request Can Also Made Through the Kiosk In-Office

If you’re already at the office, you can use the kiosk in the lobby to fill out the first form. It’s quick and easy! If you need help, just ask someone at the front desk.
